Parish Registration

Thanks for taking a moment to register your household's information with our parish office. Please fill out the form as completely as possible and then click submit. Please allow 1-2 months for your printed envelopes to arrive. If you do not wish to receive envelopes, you can join Faith Direct online giving at: faith.direct/OH641 using parish code OH641.

Registration is not complete until one meets with Fr. Philip, Fr. Joe or Deacon Mark. Please call the Parish Office to schedule an appointment, 440-734-1300. If registration is missing information, you will not be registered until registration form is fully filled out (i.e. names, address, dates of birth, sacraments received, etc.)
